Consult Coordinator/ MAT Licensed Practical Nurse
Position Summary
The Grievance Nurse monitors patients from intake to the facility and ensures continuity of care leading up to and following release into the community by advocating for patient needs while acting as a liaison between the multiple systems impacting the lives of offenders to provide a proactive and collaborative approach in serving the patient population in a correctional environment.
The Consult Coordinator helps facilitate medical consultations and appointments for patients who require specialized care beyond the facility's capabilities. They coordinate with outside healthcare providers, schedule appointments, arrange transportation, and ensure that necessary medical records are prepared. Overall, the role of a Consult Coordinator in a correctional facility involves a combination of clinical care, coordination, and administrative tasks to provide essential healthcare services to inmates while adhering to the unique challenges of a correctional environment.
The Medication Assisted Treatment Licensed Practical Nurse works as part of a team with the MAT Case Manager to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and deliver a recovery-oriented system of care.
Responsibilities
- Meets with inmates/patients regarding complaints of medical care and facilitating resolutions with the patient and medical team.
- Ability for tracking & trending data for presentation to a committee.
- Good organizational skills.
- Maintains accurate and up-to-date medical records for patients receiving specialized care. This involves documenting medical histories, test results, treatment plans, and communication with external healthcare providers.
- Maintains detailed records to ensure continuity of care and compliance with medical protocols.
- Collaborates with other members of the medical team, including registered nurses, physicians, and mental health professionals, to provide comprehensive healthcare services to inmates.
- Alongside clinical duties, may have administrative tasks such as scheduling appointments, maintaining records, and coordinating with external healthcare providers.
- Acts as a liaison between the correctional facility's medical team and external healthcare providers. They communicate medical needs, coordinate information exchange, and ensure seamless collaboration for patients' medical care.
- Accurate documentation of all medical interactions, assessments, treatments, and interventions is essential.
- Medication Inventory Management.
- Initial and ongoing SUD (substance use disorder) assessment and evaluation.
- Patient Care Coordination.
- Collaborate with community treatment providers and social support services.
- Dispense MAT Medications.
Requirements
- Graduate of an accredited School of Nursing.
- Licensure as a Licensed Practical Nurse in the state of employment.
- Possesses an active CPR certification.
- Remains knowledgeable about specific state laws and regulations governing practice.
- Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.
- Must have the ability to work independently, take initiative, and have good judgment and problem-solving skills.
