About the role
Community Healthcare of Texas provides hospice and palliative care services with a mission to offer compassionate care for individuals living with serious illnesses while supporting their loved ones. The Hospice Quality Improvement Coordinator serves as a resource for all service lines, including palliative care and hospice services, focusing on clinical staff education, quality initiatives, and regulatory compliance.
Responsibilities
- Conduct needs assessments of clinical staff and adjust training curriculum to meet identified needs.
- Monitor current regulatory requirements and best practices, incorporating them into clinical training curriculum.
- Develop curriculum, presentations, and conduct training for clinical staff.
- Coordinate clinical staff orientation and follow up with new staff to reinforce or offer further training.
- Coordinate execution of the QAPI plan, including data collection, compilation, tracking, trending, and analysis (e.g., chart audits, DEYTA reports, infection control, complaints, utilization review). Prepare reports and make recommendations based on findings.
- Conduct educational activities related to quality management and performance improvement; participate in orientation, staff development, and new leader orientation related to PI and QA.
- Assist with compliance-related activities, including maintaining knowledge of Home Care Home Base software for report generation and survey preparation, ADR responses, and routine chart audits.
- Serve as the primary resource for infection control issues, collaborating with leadership to develop and revise policies and procedures.
- Ensure compliance with Exposure and Emergency Plans, adhering to CDC and Texas Department of Health and Human Services (TDHHS) standards for infection control.
- Other duties as assigned.
Requirements
- Degree in Nursing with licensure to practice in Texas as an LVN or RN.
- Demonstrated skill in education program planning, implementation, and evaluation.
- Experience in hospice quality, including educating staff on quality measures, improvement, and compliance initiatives.
- 5 years of experience in nursing as a nurse.
- Knowledgeable of hospice regulations, standards, and infection control practices.
