About the Role
Leverages critical thinking and creative problem-solving skills to manage a caseload of 20-25 patients with complex medical and psychosocial discharge planning needs. Offers high level of autonomy in a fast-paced hospital environment, reporting directly to the Director with regular collaboration alongside physician leaders and the ACMO. Ideal candidate brings exceptional communication skills, the ability to think "outside the box," and a flexible approach to meeting evolving department needs while serving as a trusted resource to peers.
Responsibilities
- Provides comprehensive care coordination for patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Accomplishes process improvement activities for populations of patients to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Collaboratively develops and manages the interdisciplinary patient discharge plan.
- Communicates the plan across the continuum of care.
- Assists patients with discharge planning and choices by maintaining knowledge of Medicare, Medicaid, and other program benefits.
- Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team.
- Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
- Edits internal members of the health care team on case management and managed care concepts.
- Facilitates integration of concepts into daily practice.
- May supervise other staff.
Requirements
- RN: Completion of a bachelor's degree in case management or health care, current Registered Nurse (R.N.) license in state worked, and BLS certification.
- SW: Master's Degree in Social Work, Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW), or MSW with the requirement to become licensed within 6 months of hire date, and BLS certification.
Qualifications
- RN: 2 years clinical experience.
- SW: 1 year Case Manager experience in an acute care hospital.
