About the Role

This role involves self-directed work focused on delivering holistic, coordinated care to improve health outcomes, advocating for members, and collaborating with healthcare teams to ensure effective management of complex health needs.

Responsibilities

• Manage care for complex members, promoting self-sufficiency and independence.
• Conduct health risk assessments via phone or face-to-face, develop and update personalized care plans.
• Coordinate services, advocate for members, and collaborate with healthcare providers and community resources.
• Document cases timely, monitor early risk factors, and ensure holistic, person-centered care.
• Serve as primary contact for members and interdisciplinary care teams, facilitating comprehensive care management.

Requirements

• Current, unrestricted RN license in Virginia.
• 2+ years in case management, home care, or acute/rehab settings, with experience in complex medical or behavioral needs.
• Proficiency in Microsoft Office and willingness to travel within Virginia.
• Valid driver’s license, reliable transportation, and internet access for remote work.

Preferred Qualifications

• CCM certification.
• Medicare/Medicaid experience.
• Managed care background.
• Experience with complex populations.
