About the role

As a COA-accredited organization, we provide best-practice, quality case management services. You will ensure all documentation is completed per Payer, State, and COA standards, assist in marketing new referrals, and communicate with customers, colleagues, and other individuals to answer questions, disseminate information, and address complaints or concerns.

  • Make 2 contacts per week by phone or face-to-face (if no wraparound facilitator is present with MYPAC) and document accordingly.
  • Connect families with needed and available community resources.
  • Follow up with clients and agencies to document use and success of referrals.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi-weekly for up to 12 months (see MYPAC requirements above).
  • Attend and participate in weekly Treatment Team meetings.
  • Serve as a liaison with other professionals and agencies related to the client.
  • Complete a recovery support plan with all individuals within the first 30 days of referral to services.
  • Provide 24/7 on-call support for clients as per program requirements.
  • Ensure compliance with all state regulatory bodies.

Requirements

  • Bachelor’s degree in a human services discipline (e.g., Social Work, Psychology, Sociology, Counseling, Criminal Justice).
  • Experience working with children and families in case management or community resource positions.
  • Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CSS in Mississippi.
