About the role

CRi is a nonprofit organization dedicated to supporting individuals with mental health needs and developmental disabilities. The TCM program focuses on direct, person-centered services in clients' homes and communities.

Responsibilities

• Partner with Case Managers to support their caseloads
• Deliver direct, person-centered services to individuals living with mental health diagnoses
• Implement specific tasks and services outlined in Service Plans
• Complete timely, accurate documentation through service notes
• Support clients in achieving self-directed, meaningful lives with dignity and choice

Requirements

• High School Diploma required
• At least 2 years of experience working with individuals with mental health needs or diagnoses
• Ability to meet clients in the community or their homes
• Organized, responsive, and comfortable completing case documentation
