We are looking for motivated candidates to join our Guided Personal Support Program as a Community Support Coach in Reading, MA and the surrounding area. The Community Support Coach provides in-home and community support for adults with developmental disabilities. Help pave the pathway to success for individuals with developmental disabilities!Major Responsibilities
- Provides support for individuals in their home and/or within the community.
- Works on goals and skills acquisition with the individual in areas such as communication, social and recreation, ADL, safety skills while promoting independence, self-determination, and self-advocacy
- Maintains regular communication with families, Program Director and other professionals involved with the family, when appropriate.
- Works with families regarding intervention techniques if applicable.
- Affords assistance to individuals and families in accessing resources that might be beneficial.
- Respects human and legal rights of individuals served.
- Completes all necessary paperwork such as progress notes, incident reports, time sheets and required DDS progress reports throughout the year.
- Attends all annual mandatory trainings required by the program as well as Northeast Arc.
Qualifications
- High school diploma or equivalent. Bachelor's degree in human services or another related field preferred.
- Minimum 1 year of experience working with individuals with disabilities.
- Strong interpersonal skills and community skills
- Proficient technology skills for documentation, communicating, and clocking in and out while working.
- Valid driver's license
- Reliable personal vehicle required for client transportation.
- Ability to stand for periods of time.
