Community Service Provider (24362)

About the role

You will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite.

Responsibilities

• Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
• Work with family members to identify goals and objectives of treatment
• Provide services based on the needs as identified in the Treatment Plan
• Cookordinate with the child and caregiver on the scheduling and provision of services
• Participate in relevant treatment meetings and attend all required trainings
• Complete all documentation

Requirements

• Bachelor's degree in a related field and at least two years of related experience OR Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
• Valid NYS Driver's License required
