About the role
The Northwest Louisiana Human Services District seeks a Community Service Professional to provide support coordination through the Family Support Program for developmentally disabled individuals. The ideal candidate will have a minimum of six years of experience in related fields or an equivalent combination of education and experience.
Responsibilities
- Develops and implements service plans for individuals and their families, including identifying goals and appropriate services.
- Makes recommendations for support needs and monitors compliance with service plans.
- Serves as a liaison and coordinator between individuals, families, the community, and service providers.
- Prepares monthly reports, maintains records, and conducts correspondence relevant to the area of specialty.
- Prepares appeals packages and participates in appeal hearings.
- Assesses home environments and living situations to determine support needs and refer to other agencies as necessary.
- Manages caseloads of families with severely/profoundly disabled children receiving financial support.
- Ensures timely submission of supporting documents and educates families about the program requirements.
- Conducts screenings using checklists to determine the severity of the child's disability and completes agreements.
- Makes determinations for annual re-certifications and monitors documentation within the database system.
Requirements
- Minimum qualifications include six years of experience in health or developmentally disabled services, counseling, health care management, or social services; or an advanced degree in a social science field, nursing, public health, etc., with two years of experience in these areas.
- Experience substitution allows for up to 120 semester hours of college credit to substitute for up to four years of experience.
Qualifications
- A bachelor's degree with three years of experience in related fields or an advanced degree with two years of experience in these areas.
- Every 30 semester hours earned from an accredited college or university counts as one year of experience.
Skills
- Strong interpersonal skills to build relationships with clients and families.
- Ability to assess and manage complex caseloads.
- Knowledge of developmental disabilities and related services.
- Proficiency in computer systems for record-keeping and data management.

Pay
The minimum and maximum pay levels for the position are listed as follows: Community Service Professional 3 - $22.96/hour minimum - $41.38/hour maximum.
Schedule
The work schedule is from 8:00 AM to 4:30 PM, Monday through Friday.
