Community Residence Counselor - Residential DBT

About the role

The Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. This role supports individuals with mental health, substance use, or developmental conditions, helping them achieve treatment goals and maintain a safe and supportive environment.

Responsibilities

• Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
• Supervises community activities according to needs.
• Drives & transports patients to and from activities in hospital vehicles.
• Remains awake and alert, at all times, while on the job.
• Aids patients in maintaining and keeping various appointments.
• Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
• Works effectively to manage the therapeutic milieu.
• Provides patients with direction and supervision as needed.
• Fulfills established hospital & program policies, procedures, and protocols.
• Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
• Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
• Encourages patients in their activities of daily living.
• Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
• Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
• Attends and participates in rounds and case conferences when appropriate.
• Physical ability for participation in patient activities.

Requirements

• High School Diploma or Equivalent required and Bachelor's Degree in a related field of study preferred.
• One-year patient care or related experience preferred, preferably in a psychiatric setting.

Qualifications

• Driver's license (MA, CT, NH, NY, RI, VT) is required with the ability to satisfactorily meet and maintain hospital driving standards.
• Time Management, Excellent verbal and written communication skills, Ability to work as part of a team, Maintains a commitment to professional growth and competency, Basic computer skills to use a variety of electronic or online systems (PRISM).

• Knowledge of psychiatric and behavioral health principles and practices.
• Ability to communicate effectively with patients, families, and interdisciplinary team members.
• Ability to assess and document patient progress.
• Ability to manage a complex caseload.
• Ability to handle crisis situations.

$22.40 - $29.73/Hourly
