About the Role

The Community Residence Counselor (CRC) provides direct patient care and participates in the therapeutic milieu within a defined scope of practice, under the direction of the Program Manager.

This position requires patient care.

Responsibilities

  • Provides direct patient care in accordance with the current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
  • Provides and supervises community activities based on patient needs.
  • Remains awake and alert at all times while on the job.
  • Assists patients in maintaining and keeping various appointments.
  • Ensures clear, concise, and timely documentation and shift notes.
  • Establishes helpful relationships with patients while maintaining professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Manages the therapeutic milieu effectively.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital and program policies, procedures, and protocols.
  • Communicates information regarding the patient milieu and therapeutic environment to appropriate staff in a timely manner.
  • Reports observations related to medication side effects, physical, and emotional changes to appropriate medical staff promptly.
  • Encourages patients in their activities of daily living.
  • Assesses sleep patterns and assists patients in developing appropriate sleep cycles, notifying the clinical team as necessary.
  • Completes mandatory annual in-service requirements, including CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Demonstrates physical ability for participation in patient activities.
  • Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.

Requirements

  • High School Diploma or equivalent required; Bachelor’s Degree in a related field preferred.
  • Medication Administration Program Certified (MAP - Massachusetts) required by the Massachusetts Department of Public Health.
  • Class D Passenger Vehicle Driver’s License preferred.
  • One year of patient care or related experience preferred, preferably in a psychiatric setting.
  • Basic computer skills to use electronic or online systems (e.g., PRISM) for communication, clinical, and administrative purposes.
  • Familiarity with Epic preferred.
  • Ability to drive, including transporting patients, may be required based on role and location.
