Ensure that physician orders are present, current and inclusive of all required information per Agency and DMH policies. Ensure that physician orders are signed by the prescribing physician before expiration date.Review medical records (physician orders, MARs, visit reports) on a routine basis. Respond with appropriate action when issues arise.Maintain and keep current the EMAR (Electronic Medication Administration Record) as dictated by physician’s orders. Review staff entries in Medication Administration Records for thoroughness and accuracy. Coach staff as needed to correct any trending errors found.Respond to phone calls and emails in a timely manner and participate in the RN On-Call Rotation.Complete face-to-face assessments with residential clients as required by individual client medical need with a minimum of one face-to-face visit per month to ensure health and identify signs and/or symptoms of disease processes.Train and delegate duties to direct support staff as nursing standards allow. Routinely check staff for competency. Maintain accurate records of delegations performed.Train, mentor and guide agency staff through Level 1 Med Aide training.Act as a liaison between UCP Heartland and medical providers to ensure appropriate client care.Act as a medical expert providing input to client’s medical care while effectively communicating with the team. Attend ISP Meetings, transition Meetings and other meetings where individual care needs will be discussed. Complete all required documentation by the deadline identified by the agency or funder requirements.Update and keep current the HRST (Health Risk Screening Tool) system as required. Generate Health Risk Support Plans as indicated by assessmentsOther duties as assigned(This is NOT a supervisory position. Agency/Community RN is supervised by Director of Health Services) MINIMUM EMPLOYMENT STANDARDS Registered Nurse with an active license with the State of MissouriDemonstrates written and verbal communication skillsOne year of experience as a practicing registered nurseStrong medical assessment skillsDemonstrates working knowledge of basic computer softwareAbility to work independently and as part of a teamExceptional time management with demonstrated ability to consistently meet deadlinesAvailable for flexibility in work scheduleReliable Transportation with Valid Driver's License Preferred Employment Standards Three or more years of experience working with people with developmental disabilities
