About the Role

The Community RN Coordinator supports the CHS mission by promoting the physical and mental health of individuals through the application of nursing processes. This role ensures qualified staff through the delivery of Level 1 Medication Administration and CPR/First Aid instruction.

Responsibilities

  • Promotes the physical and mental health of individuals by:
    • Coordinating and monitoring medical services in accordance with regulatory requirements and individual needs.
    • Providing at least 1.25 hours of service to each individual supported each month.
    • Participating in the admission of new individuals to assigned programs.
    • Clarifying medical information and advocating for individuals during hospital admissions and/or discharges.
    • Communicating with healthcare providers or pharmacists to review individual health statuses and medication plans.
    • Conducting full health assessments (including skin assessments) within 7 days of discharge from hospitalization or ER visits.
    • Coordinating ongoing care with clinics, external healthcare providers, hospitals, and pharmacies.
    • Reviewing and coordinating lab work as required.
    • Traveling as needed to provide medical-related support to individuals.
    • Reviewing and teaching health and nutrition per individual needs.
    • Conducting Professional Assessment and Monitoring services as authorized.
    • Promoting individual empowerment and self-care activities, including self-medication.
    • Participating in on-call rotations with other community nursing staff.
  • Ensures medically qualified staff by:
    • Providing nursing delegations related to medical diagnoses, allergies, medication, diet, and special emergency plans.
    • Becoming certified to instruct Level 1 Medication Administration and CPR/First Aid training.
    • Developing instruction methods and selecting teaching aids (e.g., handbooks, study aids, demonstration models, visual aids, online tutorials).
    • Conducting in-person, recorded, or virtual training using diverse instructional methods (e.g., individual training, group instruction, lecture, demonstration).
    • Testing trainees to measure progress and effectiveness.
    • Reporting progress of employees in training to departmental management.
    • Supervising Community Registered Nurse (RN) services as required.
  • Communicates complex medical information to individuals, staff, and stakeholders by:
    • Developing plans to ensure the medical needs of individuals are met.
    • Following up on health-related concerns identified by the Department of Mental Health (DMH) RN.
    • Completing the medical sections of Individual Support Plans (ISPs).
    • Participating in departmental and other staff meetings to ensure alignment and knowledge transfer.
    • Answering questions about the healthcare and condition of individuals with staff, guardians, service coordinators, and the individuals themselves.
  • Supports regulatory compliance by:
    • Documenting services in accordance with agency policy, Medicaid, DMH certification, and other contractual requirements.
    • Conducting regular reviews of medication administration documents for accuracy and completion.
    • Ensuring timely completion of CIMOR and APTS referrals.
    • Inputting and updating information in the state HRST system monthly.
    • Maintaining current knowledge of company and departmental policies and procedures.
    • Protecting confidentiality and following HIPAA guidelines.
    • Maintaining licensing and certifications necessary for work.
    • Participating in continuing education to remain current with best practices.
    • Acting as a local medical advisor for infectious disease control or health crises per policy and protocols.
  • Promotes a positive work culture by:
    • Supporting other community nurses with a team spirit, including coverage for absences to preserve continuity of care.

  • Ensures staff success and regulatory compliance by:
    • Keeping Community RNs informed of individuals’ plans and health-related updates.
    • Administering corrective actions as necessary and providing constructive, timely feedback.
    • Offering coaching and mentoring to support staff growth and foster collaboration.
    • Holding RNs accountable for accurate, complete, and timely documentation.
    • Ensuring RNs remain current with regulatory requirements and demonstrate thorough knowledge of individuals’ ISPs.
    • Maintaining up-to-date knowledge of all relevant regulations, rules, policies, and procedures.

Requirements

  • Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
  • Current State License or Compact State License as a Registered Nurse.
  • Must obtain and maintain Professional CPR certification.
  • Must obtain and maintain DHSS Approved Instructor certification.
  • Valid driver’s license.
  • Reliable, licensed, and insured vehicle.
  • Reliable form of communication (e.g., smartphone).

Physical Requirements

  • Ability and strength to position/lift/transfer over 50 lbs. (e.g., assisting an adult human being), including safely assisting an adult to the ground to perform CPR.
  • Continuous ability to push and pull 100-125 lbs. of pressure consistently over extended periods (e.g., performing CPR as defined by the American Heart Association).
  • Continuous ability to kneel on hard or other surfaces for extended periods (e.g., performing CPR).
  • Ability to type on a keyboard for long periods.
  • Vision sufficient to read medication labels, dosing instruments, and/or view computer screens for extended periods.
  • Ability to drive distances of 200+ miles.
