About the Role

The Community Program Coordinator is responsible for the oversight, coordination, and quality management of Community Participation Support (CPS) and In-Home and Community Support services for individuals with Intellectual Disabilities (IDD), Developmental Disabilities (DD), and Autism Spectrum Disorder (ASD). This position ensures the delivery of person-centered services that promote independence, community inclusion, skill development, and achievement of individual goals while maintaining compliance with agency, state, and funding source requirements.

Responsibilities

  • Oversee, coordinate, and manage Community Participation Support (CPS) and In-Home and Community Support services.
  • Ensure delivery of person-centered services promoting independence, community inclusion, and skill development.
  • Maintain compliance with agency, state, and funding source requirements.
  • Support individuals, families, and communities to achieve individual goals.

Qualifications

Required:

  • Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, or related field preferred.
  • Experience working with individuals with IDD, DD, and/or ASD.
  • Knowledge of Community Participation Support (CPS), In-home and Community Services, Individual Support Plans (ISPs), documentation standards, and billing requirements.
  • Must become ACRE Certified or CESP Certified within 6 months of hiring.

Preferred:

  • Minimum of three (3) years of supervisory or management experience.
  • Experience managing Employment Services, Community Participation Support (CPS), or In-Home and Community Supports (IHCS) programs.
  • Strong organizational, communication, and leadership skills.
  • Ability to manage multiple priorities while maintaining compliance and quality standards.
  • Proficiency in electronic health records, documentation systems, and Microsoft applications.

Physical Requirements

  • Ability to sit, stand, walk, bend, stoop, reach, lift, and climb stairs as needed.
  • Ability to drive a vehicle and travel frequently within assigned service areas.
  • Ability to operate standard office equipment and electronic documentation systems.
  • Ability to perform essential job functions with or without reasonable accommodation.
