Community Outreach Specialist
We are a mission-driven nonprofit organization in Santa Clara, CA seeking a compassionate, organized professional to support Enhanced Care Management services for adults, caregivers, and families navigating complex health and social service needs. About the role
As a Community Outreach Specialist, you will play an important role in outreach, intake, enrollment, referrals, and participant engagement. This position is ideal for someone who enjoys community engagement, client support, and connecting individuals with meaningful resources.
Responsibilities
- Perform telephonic and in-person outreach to engage potential Enhanced Care Management participants and explain available program benefits.
- Meet with participants in community, office, and other approved settings to support intake, enrollment, consent collection, and engagement.
- Use research tools, provider contacts, referral lists, and health plan information to locate and follow up with eligible participants.
- Coordinate referrals to community-based, social support, and health-related resources based on participant needs.
- Maintain accurate documentation, data entry, reporting, and compliance records in accordance with confidentiality and program requirements.
- Participate in community outreach events, presentations, audits, and other program support activities as assigned.
Requirements

- Experience in outreach, intake, enrollment, or care coordination preferred.
Pay
$28.00 to $31.00 per hour.
Schedule
Hybrid schedule with 4 days in office and 1 day work from home once training is complete. Planned local travel during working hours as needed for in-person outreach and community events.
