About the role

This role operates in a Day Habilitation (DAY HAB) Setting, providing support for community inclusion and building skills for active participation in various activities.

Responsibilities

  • Collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.
  • Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction rather than doing tasks for them.
  • Follow individualized behavioral care plans by applying approved strategies, encouraging participation, motivating progress, and supporting positive behavior in alignment with each person’s goals.
  • Maintain accurate, consistent, and timely documentation of individual progress, activities, and outcomes to ensure continuity of care and accountability.
  • Provide a safe, supportive environment while respecting individual choice and autonomy, advocating for independence, dignity, and person-directed decision-making in all aspects of service delivery.

  • Work authorization: Able to work in the U.S. without sponsorship.

Physical Demands

  • Lifting up to 35 pounds, requiring the use of assistive equipment for more than 35 pounds.
  • Prolonged sitting and working at a computer for up to eight hours.
  • Frequent bending, stooping, and stretching.
  • Prolonged standing for up to eight hours.
  • Wearing hearing and eye protection in required areas.
  • Ability to see objects/persons at a distance, close work, conversations in a quiet and noisy environment, determine sound sources, and communicate through speech.
  • Ability to drive and transport for service provision if needed.
  • Remaining awake during working hours, if applicable.
