Community Integration Counselor (HCBS)

About the role

The Community Integration Counselor (CIC) at Community Options, Inc. is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills. The CIC provides individualized services designed to help participants manage and overcome challenges associated with living in the community.

Responsibilities

• Support and encourage individuals to increase their independence
• Meet with waiver participants regularly in various settings
• Document individual progress towards community integration
• Aid in building a support network between individuals and the community
• Administer medications and ensure proper documentation
• Coordinate leisure and volunteer opportunities
• Monitor health and medical needs, reporting any concerns immediately
• Drive individuals to medical appointments and activities in the community
• Ensure accurate and timely completion of program documentation and billable records
• Perform additional tasks as assigned

Requirements

• High school diploma or GED
• Valid driver’s license with a satisfactory driving record
• Effective communication skills in English
• At least 3-6 months of relevant experience supporting individuals with intellectual and developmental disabilities
• Ability to establish a comfortable and supportive relationship with individuals receiving supports

Qualifications

• Passing background checks, including criminal background, central registry, and child abuse registry
• Passing drug testing
