Community Health Worker, State and Federal Grants, Jackson Main, FT, Days

Responsibilities

• Carry out community outreach, enrollment, and home visits to educate and assist clients.
• Identify and address barriers to care, and connect clients with necessary resources.
• Arrange transportation to medical appointments and other community resources as needed.
• Collaborate with social workers, nurse navigators, and other team members to ensure comprehensive care.
• Keep client files updated and maintain ongoing communication with clients.
• Aid in the development of programs to encourage clients to utilize social or health services.
• Maintain relationships with community resource agencies to stay informed about referral methods.
• Participate in meetings with other staff to discuss client needs and family dynamics.
• Attend relevant in-service training sessions.

Requirements

• High school diploma is required.
• Generally requires 0 to 3 years of related experience.
• Ability to analyze, organize, and prioritize work accurately while meeting multiple deadlines.
• Effective communication skills in both oral and written forms.
• Ability to handle difficult and stressful situations with critical thinking and professionalism.
• Knowledge and proficiency in using job-appropriate technology and software applications.
• Must meet and maintain current unit-specific and organizational skills/competencies, certifications, and licenses, as required by the nursing area.
