Community Health Worker - PT Days

About the Role

The Community Health Worker (CHW) will provide access to preventive and medical care services for patients in ambulatory and inpatient settings. CHWs offer direct linkage to supportive resources that address and better support social determinants of health.

Responsibilities

• Provide screenings of risk factors that impact an individual’s access to care.
• Follow up with patients by telephone to connect them to medical appointments and answer questions about recommended next steps.
• Build relationships with patients by telephone to connect them to medical appointments and answer questions about recommended next steps.
• Build and maintain relationships with existing business partners that support business office functions.
• Follow up and assist families who are having difficulty accessing resources to which they were referred.
• Fully understand and explain the issues for which they are screening and the resources available to meet those needs.
• Understand and explain insurance and CalFresh programs (such as eligibility, benefits, application process, requirements, premiums, and co-pays) and other community health services.

Requirements

• Driver’s License, auto insurance, and reliable transportation.

Pay

$25.00/hour
