Position Summary

Community Health Workers (CHWs) are flexible members of a health care team who work with vulnerable populations to improve health outcomes. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.

Education And Experience

  • High School Diploma or equivalent required
  • Bachelor’s Degree in social work, psychology, counseling or related human service field from a college/university accredited by the US Department of Education preferred
  • One (1) year of related experience preferred

Certification

Must be able to achieve and maintain Community Health Worker certification within 12 months of hire

  • Must be able to work independently as well as collaboratively within a team setting

  • Exhibit ability to work with targeted population, and possess knowledge of and ability to relate to community resources
  • Able to work well under pressure in a fast-paced, time sensitive environment with shifting priorities and multiple deadlines
  • Possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must have reliable transportation and utilize personal vehicle to transport self/others and to attend trainings and/or meetings
