Community Health Worker – Foundational Community Supports (Newport, WA)

Position Summary

Responds to clients in need of services and supports. Provides aid needed to help qualified clients to obtain and maintain safe and stable housing. Works closely with the Program Development Manager to ensure project success and effective communication amongst all necessary program components. Maintains a timely and accurate record of responsibilities and completed tasks. Conducts housing supportive work with clients and helps to address social determinants and unmet social needs.

Essential Job Functions

Conduct in-person visits with clients across designated service areas, requiring frequent travel by vehicle. This includes driving to client homes, community settings, and partner agencies to provide support.
