About the role

The Community Health Worker (CHW) position at Spectra Health provides direct care services to patients. The CHW bridges the gap between healthcare providers, social services, and the communities Spectra Health serves. Key responsibilities include responding to provider referrals, conducting home visits, coordinating patient care, and advocating for clients.

Responsibilities

  • Respond to provider referrals and complete resource assessments
  • Develop and implement care/service plans
  • Monitor and evaluate care plans to ensure they meet patient needs
  • Communicate with providers, staff, and community agencies to coordinate care
  • Conduct home visits and community outreach to assess health needs and provide guidance
  • Advocate for clients by identifying barriers to care and connecting them with resources
  • Assist with or conduct outreach efforts on behalf of Spectra Health
  • Provide health system navigation and resource coordination
  • Offer health promotion and coaching, and provide health education and training
  • Aid in coordinating and enrolling patients in Spectra Health's sliding fee program
  • Communicate with patients regarding billing and insurance issues
  • Place follow-up calls to patients as necessary
  • If appropriate, complete annual Certified Application Counselor (CAC) training and certification
  • Willingness to accompany patients to appointments and complete paperwork as requested
  • Ability to work with Certified Interpreters regularly
  • Ability to comply with data collection requirements
  • Ability to work in both Larimore and Grand Forks locations

Requirements

  • Be a trusted member of the community served
  • Minimum of a high school diploma or equivalent
  • CHW Certification in ND or willingness to obtain within six months of employment
  • Valid Driver's License, licensed and insured vehicle
  • CPR/BLS certification within six weeks of employment

Qualifications

  • Bachelor's Degree in Social Work or a related field preferred
  • Credentialed as an LBSW (Licensed Baccalaureate Social Worker) in North Dakota preferred
  • Experience working in a clinical or hospital environment preferred
  • Experience working with homeless and transient populations preferred
  • Experience working with New American/Refugee populations preferred
  • Experience working with interpreters preferred
  • Ability to speak multiple languages preferred
