About the role

The Community Health Worker (CHW) at INTEGRIS Health in Enid, OK, works with patients in the hospital and in the community to help them get the care and services they need. The CHW serves as a trusted link between patients, the hospital care team, and community organizations.

Responsibilities

  • Screen patients for social needs, including food insecurity, housing concerns, transportation barriers, utility needs, personal safety, and nicotine use (including youth ages 13–17).

  • Identify patients who may benefit from chronic disease prevention or management programs, such as weight management, diabetes prevention, or tobacco cessation.

  • Use approved screening tools and patient-centered communication techniques.

  • Document all screenings and patient interactions in the electronic health record (EHR) and other required systems.

  • Connect patients to hospital services and community organizations.

  • Help patients apply for and enroll in public benefit programs such as SNAP, Medicaid, and My Life My Quit.

  • Follow up with patients to make sure they were able to access services and address any barriers.

  • Support discharge plans by reinforcing medication instructions, follow-up appointments, and transportation needs.

  • Work with care managers, social workers, nurses, and providers to support patient care plans.

  • Track referral outcomes and complete required internal reports, including monthly reports to the Oklahoma Hospital Association.

Qualifications

  • Associate’s degree required; Public Health or related field preferred.

  • Bachelor’s degree preferred; Public Health or related field a plus.

  • Experience using electronic health record systems.

  • Bilingual or multilingual skills that reflect the community served.

  • Experience working in rural healthcare settings.

  • Hospital-based setting with some community outreach.

  • May require travel within the hospital’s service area.

  • May include occasional evening or weekend events.
