Salary Range: $18.00-22.00/Hour

About the role

We are currently hiring a Community Health Worker based in Milford, MA. The CHW is a key member of the health care team, providing comprehensive and coordinated interventions to address the social determinants of health (SDOH) as part of a patient-centered medical home.

Responsibilities

• Conduct psycho-social assessments with patients referred for Community Health Worker interventions in accordance with Health Center policies, procedures and protocols
• Provide health system education and advocacy to assist patients in navigating the health care system
• Provide basic education, screening, brief intervention and/or facilitate referrals to other health center or community-based resources in accordance with applicable health center protocols

Job Requirements

• Basic knowledge of Microsoft Office
• Able to learn the health center’s Electronic Medical Record and other applications
• Successful completion of the CHW core competency training within 2 years of employment
• BLS Certification, can be obtained through the Health Center
