Job Summary
Hybrid Work Environment - Must reside on Oahu
Pay Range: $47,500 - $88,000
Note: Individuals typically begin between the minimum to middle of the pay range
About the Role
Serves a critical role in advancing health and wellness within the community by fostering trust, advocating for individuals and families, and facilitating timely access to appropriate care.
Responsibilities
- Performs outreach, offers educational resources, coordinates care, supports closing care gaps, and connects members with relevant community services to address social determinants of health (SDOH).
- Collaborates with healthcare and community professionals to optimize health outcomes, member satisfaction, and overall community well-being.
- Assists with and/or reviews comprehensive assessments completed by the RN—including physical, psychosocial, and health literacy evaluations, and related care plans.
- Ensures timely and thorough documentation of all care activities per federal, state, and NCQA requirements.
- Reports findings, concerns, and significant developments.
- Engages in quality improvement initiatives.
- Analyzes dashboard data for program enhancement.
Requirements
- Associate's degree and three years relevant healthcare/social services experience; or equivalent education/work experience.
- Fieldwork experience such as home visits, conducting health screenings in the community, and/or navigating social services (like food, housing).
- Excellent communication skills, both written and verbal.
- Basic working knowledge of Microsoft Office applications, including but not limited to Word, Excel, and Outlook.
- Must have reliable transportation for in-person meetings with members at home or other locations.
Qualifications
