About the Role

This position provides support to the Community Health Division through outreach, education, and care coordination. It delivers health promotion and disease prevention information within the community to reach medically underserved populations and encourage healthy behaviors. This role is designated as emergency essential.

Responsibilities

• Conduct community outreach and education on health care and disease prevention.
• Provide care coordination services to medically underserved populations.
• Promote healthy behaviors and disease prevention within the community.

Requirements

• Graduation from an accredited high school or possession of a high school equivalency certificate.
• One year of experience providing information on health care and disease prevention within the community.

Notes:

• Candidates may substitute experience providing assistance to individuals in a health care or social services setting on a year-for-year basis for the required education.
• Candidates may substitute U.S. Armed Forces military service experience as a non-commissioned officer in Health Services classifications or Health Services specialty codes in the health-related field on a year-for-year basis for the required experience.

Qualifications

Preferred qualifications include:

• Experience with Microsoft Office and Google services.
• Experience with electronic health records.
• Experience with public speaking.

Licenses and Certifications

• Employees assigned duties requiring the operation of a motor vehicle must possess a valid Maryland motor vehicle operator's license.
