Community Health Navigator

About the role

At Reema Health, we believe in a personalized, community-based approach to care. Our Community Health Navigators, or Community Guides, build genuine relationships with members to guide them toward better health outcomes.

Responsibilities

• Conduct phone and in-person outreach to enroll members in Reema's care model
• Manage a caseload of 80-120 members, covering for other guides as needed
• Assess member needs, including social determinants of health, to build a member-centered care plan
• Support members in achieving their health goals, addressing barriers with empathy and follow-through
• Coordinate referrals and support appointment scheduling and attendance
• Maintain professional boundaries and confidentiality; consult the multidisciplinary team on complex situations
• Document all member work thoroughly and on time in the Reema App
• Represent Reema Well with integrity and respect, building relationships with community resources

Requirements

• 18 years of age or older
• Available to work Monday–Friday during core business hours
• (If field-based:) Valid driver's license, insurance, and reliable transportation
• Pass a pre-employment drug screening and background check
• Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA
• Use technology-based tools and systems daily
• Lift and carry up to 25 lbs occasionally

Qualifications

• 1+ years of direct experience with the population/community served, or equivalent bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field
• Experience with crisis planning, case management, and professional boundaries in community-based work
• Relationship-building, community resource knowledge, and health system navigation
• Comfort working as part of a multidisciplinary team
