Community Habilitation/Integration Specialist

Responsibilities

• Deliver therapeutic services including individual, group, and family therapy
• Provide timely crisis intervention and emotional support
• Collaborate with a multidisciplinary treatment team to ensure coordinated care
• Complete initial treatment plans within 30 days of admission
• Update treatment plans every 180 days and maintain weekly progress notes
• Prepare discharge summaries and ensure smooth transitions out of care

Requirements

• Master's level clinician with a degree in social work, mental health counseling with diagnostic privilege (LMHC,LMSW); Licensure or limit-permit required
• Bilingual in the Spanish language required
• Minimum one year experience providing clinical assessments/treatments, and/or case management services for children/adolescents and families
• Knowledge of the complex issues presented by children and families who are involved in the child welfare system
• Have some working knowledge of NYC systems including ACS, OMH, and DOE
• Familiarity with OMH-Mental Health Outpatient Treatment and Rehabilitative Service programs operated clinics, Part 599 regulations is preferred

Pay

Pay Rate: $60 per hour

Schedule

Position Hours: Hours include evenings and Saturdays; Clinical Service delivery will occur both on-site and via telehealth.
