Community Care Nurse Case Manager (Full Time)

Responsibilities

• Member of the multi-disciplinary Care Management Team that provides individualized member-driven planning, coordination, assessment, and monitoring of services to help enrolled Montana Big Sky Waiver Medicaid beneficiaries who are elderly or physically disabled residing in their own homes or communities.
• Provides clinical assessment, medication review, management of chronic illnesses, and coordination of skilled services that focus on the health service clinical needs, medical stability, and skilled services of the member.
• Prevent hospitalization and ensure medical stability at home.
• Deal with pressure to meet deadlines, be accurate, and handle constantly changing situations.
• Deal with a variety of people, stressful situations, and handle conflict.

Requirements

• Graduate of an accredited school of nursing, bachelor in nursing preferred or LPN, if state approved.
• Current state licensure as a Registered Nurse or LPN if state approved.
• Certification in Case Management preferred.
• Experience as a Nurse (RN/LPN) commensurate with minimum of three (3) years of professional nursing experience.
• Knowledge of case management methods, practice, and procedure.
• Knowledge of the application of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.
• Prior knowledge of managing a budget.
• Prior experience with Medicare and Medicaid reimbursement.
• Knowledge of human behavior, disabilities, and the aging process.

Qualifications

• Must live in or around Gallatin County Monday - Friday No Weekends or call.

Pay

Starting Wage $27.31/hourly Actual offered wage is based on applicable experience.
