About the role
Provide community-based care coordination to individuals with substance use, behavioral health symptoms/diagnosis, and/or those experiencing employment, legal, or housing issues. Services will focus on meeting individual needs from a holistic, integrated strengths-based model to enhance individual’s cognitive, social, emotional, and coping skill sets. Services are designed to reduce risk, gain access to needed resources, increase stability, and improve one’s overall health.
Responsibilities
- Serve as a central point of contact for clients
- Work to link clients to resources in the areas of Recovery, Housing, Employment, Mental Health and Physical Health
- Reduce barriers and facilitate effective communication in these areas
- Help clients regain stability in their lives and prevent or reduce the need for hospitalization, detoxes, or jail admissions by directing them toward beneficial community resources
- Focus on the whole person and stress comprehensive assessment, service planning, and service coordination to address multiple aspects of a client’s life
- Use a Strengths-Based Case Management Model, Motivational Interviewing, and Brief Intervention Skills
- Advocate for client needs to increase opportunities for self-sufficiency
Requirements
- Minimum qualifications: Masters or Bachelor level preparation with experience in social work, health care related field, and behavioral health or substance abuse systems is preferred.
- Licensure through the Behavioral Sciences Regulatory Board of KS preferred.
- LAC – Licensed Addictions Counselor
- LMAC – Licensed Masters Addictions Counselor
- LCAC – Licensed Clinical Addictions Counselor
- Candidates in process of obtaining BSRB licensure will also be considered.
- Computer skills: Proficiency with Microsoft Office Suite including Word, Excel & Outlook.
- Documentation and reporting: Attention to detail required.
- Autonomy and stakeholder communication: Ability to work autonomously and effectively communicate with stakeholders, clients, and staff.
- Language skills: Bi-lingual skills a plus.
Qualifications

- Interpersonal skills: Excellent interpersonal skills required – both verbal & written.
