Collaborating Physician

About the role

Physician collabs earn a monthly stipend based on the number of Signify Health-contracted nurse practitioners and/or physician assistants they oversee. You must be licensed in and living in Alabama in order to qualify.

Responsibilities

• Perform in-home health evaluations for health plan members.
• Conduct medication and medical history reviews.
• Perform physical evaluations in a health plan member’s home.
• Use a pre-populated, patient-specific form for each evaluation.
• Provide referrals to the individual’s health plan when necessary.
• Encourage participation of loved ones in the member’s care.
• Ensure the member’s primary care physician is aware of their health and overall quality of life.

Requirements

• Doctorate (Required)
• AL Medical License (Preferred)
• Must be licensed in and living in Alabama

• Flexible schedule based on your preferences and availability

Compensation

• From $100.00 per hour
• Additional compensation opportunities via a pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
