About the role
Interim HealthCare, one of the nation’s leading providers of home care, is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. As a Hospice CNA, you will play a crucial role in delivering high-quality, personalized end-of-life care to patients in their homes, ensuring their comfort, dignity, and overall well-being during this sensitive phase of life.
Responsibilities
- Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and monitoring vital signs.
- Document any changes in the patient's condition and report concerns to the registered nurse or hospice team.
- Assist in administering medications as per prescribed treatment plans and report any adverse reactions or side effects.
- Maintain accurate records of medication administration.
- Maintain a clean, safe, and comfortable environment, making necessary adjustments to ensure patient well-being.
- Implement appropriate pain management techniques as outlined in the patient's care plan.
- Maintain detailed patient records, documenting care provided, observations, and any changes in the patient's condition.
- Ensure compliance with all documentation and reporting requirements as per hospice regulations.
- Work collaboratively with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to deliver holistic care to patients.
- Participate actively in team meetings and contribute to care planning discussions.
Qualifications
- Licensed CNA in the State of Texas – required
- 1+ years' experience – preferred
- Experience with hospice or home health – preferred
- Effective written and verbal communication
- Ability to travel – must have reliable transportation
- Clean background check and drug screen
