About the role
The primary purpose of this position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as directed by supervisors.
Responsibilities
- Identify residents using wristband or photo card before administering treatments or serving meals.
- Use only authorized facility abbreviations when recording information.
- Report changes in a resident’s condition to the Nurse Supervisor/Charge Nurse promptly.
- Record entries on flow sheets, notes, or charts in an informative, descriptive manner.
- Report all accidents and incidents observed during the shift immediately.
- Agree not to disclose residents’ protected health information and report suspected violations.
- Report any known or suspected unauthorized attempts to access facility information systems.
- Perform only nursing care procedures for which you have been trained.
- Prepare residents’ rooms for receiving them (e.g., make beds, place name tags, prepare admission kits).
- Greet residents, escort them to their rooms, and introduce them to roommates and staff as appropriate.
- Make residents comfortable and assist with personal care needs (e.g., dental/mouth care, bathing, dressing).
- Inventory and mark residents’ personal possessions as instructed; store clothing and assist with packing during transfers or discharge.
- Transport residents to new rooms or receiving areas and assist with loading/unloading from vehicles.
- Create and maintain a warm, positive, and calm environment on the unit and shift.
- Meet regularly with shift nursing personnel to identify and correct problem areas or improve services.
- Report all resident complaints and grievances.
- Participate in and receive nursing reports as instructed.
- Follow established policies concerning exposure to blood and body fluids.
- Make beds, add extra covers as requested, and ensure residents who cannot call for help are checked frequently.
- Answer resident calls promptly and check routinely to ensure personal care needs are met.
- Assist residents with identifying food arrangements (e.g., describe food placement and temperature for visually impaired residents).
- Record food and fluid intake; report changes in eating habits to the Nurse Supervisor/Charge Nurse.
- Keep residents’ water pitchers clean, filled with fresh water each shift, and within easy reach.
- Perform after-meal care (e.g., remove trays, clean residents’ hands, faces, and clothing).
- Follow all assigned tasks in accordance with established policies, procedures, and supervisor instructions.
- Cooperate with inter-departmental and facility personnel to maintain adequate nursing services.
- Notify the facility of anticipated lateness or absence from work.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies.
- Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
- Assist residents in preparing for medical tests (e.g., lab work, x-ray, therapy, dental).
- Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
- Perform restorative and rehabilitative procedures as instructed.
- Assist in preparing residents for physical examinations.
- Observe and report pressure areas, skin breakdowns, skin tears, and injuries of unknown source.
- Observe disoriented and comatose residents and report observations as instructed.
- Perform special treatments as instructed.
- Immediately notify the Nurse Supervisor/Charge Nurse if a resident is missing or leaves the facility.
- Use only equipment for which you have been trained; report defective equipment to the Nurse Supervisor/Charge Nurse.
- Maintain confidentiality of all resident care information, including protected health information.
- Report known or suspected unauthorized disclosures of resident information.
- Report all allegations of resident abuse and/or misappropriation of resident property.
- Honor residents’ refusal of treatment and report such refusals to supervisors.
Qualifications
- Minimum education: 10th grade education.
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to present information and respond to questions from managers and employees.
- Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
- Ability to solve practical problems and deal with concrete variables in non-standardized situations.
- Ability to interpret instructions furnished in written, oral, diagram, or schedule form.
- Must be a licensed Certified Nursing Assistant in accordance with state laws.
- CPR certification preferred.
Physical Demands
- Occasionally lift and/or move up to 25 pounds.
- Prolonged use of desktop or laptop computers.
- Regularly required to sit, stand, walk, and talk; read or hear.
- Frequent use of office equipment including copier/scanner/fax, telephone, and calculator.
- May assist in evacuating residents during emergency situations.
Work Environment
- Noise level in the work environment is usually low to moderate.
