Responsibilities
- Assist residents with daily activities including bathing, dressing, grooming, toileting, oral care, and other personal needs.
- Absorb and document vital signs, weights, intake/output, and other resident information as assigned.
- Observe and promptly report changes in residents' condition, behavior, skin integrity, mobility, appetite, or overall well-being.
- Follow individualized care plans and provide restorative and rehabilitative activities as directed.
- Absorb and document residents' meals, including feeding assistance and documenting food and fluid intake as directed.
- Absorb and document transfers, repositioning, ambulation, mobility, and use of mechanical lifts and other equipment.
- Absorb and document therapy, appointments, activities, admissions, transfers, and discharges.
- Respond to resident call systems and routinely check on residents to ensure their needs are being met.
- Maintain accurate documentation and communicate important resident information to nursing staff.
- Maintain resident dignity, privacy, confidentiality, and rights at all times.
- Follow infection prevention, safety, emergency, and facility procedures.
- Keep resident rooms, care areas, and equipment clean, safe, and organized.
- Participate in required training, education, and in-service programs.
- Work collaboratively with nursing, rehabilitation, social services, dietary, activities, and other departments.
- Perform other duties as assigned.
Qualifications (CNA - Certified Nursing Assistant)
- Seventeen (17) years of age required; high school diploma or equivalent preferred.
- Employment Certificate/Permit required for 17 years of age.
- Registration in the State Nurse Aide Registry (CNA) is required.
- Active on the State Nursing Aide Registry with no findings or convictions of resident abuse, mistreatment or misappropriation of property.
- Training and work experience in a geriatric setting preferred.
- Ability to supervise and work well with other personnel.
- Ability to communicate well verbally and in writing.
- Caring and respectful attitude towards residents.
- Physical stamina for constant activity required.
- Ability to perform tasks to established standards of excellence required.
