Responsibilities
- Follows the Care Plan to assist the patient in maintaining a healthful, safe environment
- Performs ONLY those functions specified for each individual patient on the Care Plan
- Receives written instructions from the regional scheduler via AxisCare Mobile App
- Has knowledge of agency policies and procedures
- Is oriented and trained in all aspects of services to be provided to patients
- Is able to demonstrate competency in all areas of training for patient services
- Assists persons at home in managing and maintaining a household, dressing, meal prep and incidental household tasks
- Personal care: bathing, grooming, feeding, ambulation, exercise, oral hygiene and skin care
- Assistance with medications ordinarily self-administered as assigned
- Household services essential to care in the home
- Completion of records and reporting to the appropriate supervisor
- Charting intake and output
- Reports any change in the patient's mental or physical condition or in the home situation to his/her immediate supervisor or Administrator
- Completes patient care-log in AxisCare Mobile App at the time of service, per agency policy
Job Conditions
- The ability to drive and the ability to access patients’ homes which may not be routinely wheelchair accessible are required
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient services
- On occasion, may be required to bend, stoop, reach; lift and/or carry up to 50 pounds
- Must be able to communicate clearly, both verbally and in writing in English; understand and carry out directions and instructions, record messages and keep simple records
Company Information
Has access to all patient medical records which may be discussed with the supervisor or relevant personnel only.
Qualifications
- Knows how to read and write English and carry out directions
- Must pass a state and/or federal background check and is eligible to receive a DHS Practitioner Identification Number
- Has a certificate issued by the State of Arkansas for working in long-term care facilities, or
- Has completed a 40 hour aide training course that meets the requirements in the Arkansas Rules and Regulations for Private Care Agencies
- Complete annual 12 hour in-service training
