About the role

Provide each assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as directed by supervisors.

Responsibilities

  • Use wristband or photo card file to identify residents before administering treatments, serving meals, etc.
  • Use only authorized abbreviations when recording information.
  • Report all changes in resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
  • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
  • Report all accidents and incidents observed on the shift that they occur.
  • Maintain confidentiality of resident protected health information and promptly report suspected or known violations.
  • Report any known or suspected unauthorized attempt to access facility's information system.
  • Perform only those nursing care procedures that you have been trained to do.
  • Prepare resident's room for receiving resident (bed made, name tags up, admission kit available, etc.).
  • Greet residents and escort them to their room; introduce to roommate and other residents/personnel as appropriate.
  • Make resident comfortable; inventory and mark resident's personal possessions; store resident's clothing.
  • Assist residents with packing personal possessions when being transferred or discharged.
  • Transport residents to new rooms or receiving area; assist with loading/unloading to/from vehicles as necessary.
  • Create and maintain atmosphere of warmth, personal interest, positive emphasis, and calm environment throughout unit and shift.
  • Meet with shift nursing personnel regularly to identify and correct problem areas and improve services.
  • Report all complaints and grievances made by the resident.
  • Participate in and receive nursing report as instructed.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds; put extra covers on beds as requested.
  • Measure and record temperatures, pulses, and respirations (TPRs) of residents.
  • Weigh and measure residents as instructed.
  • Ensure residents who are unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are being met.
  • Assist residents with identifying food arrangements (informing resident with sight problems of foods on tray, location, temperature, etc.).
  • Record resident's food/fluid intake; report changes in eating habits to Nurse Supervisor/Charge Nurse.
  • Keep residents' water pitchers clean, filled with fresh water each shift, and within easy reach.
  • Perform after meal care (remove trays, clean resident's hands, face, clothing, etc.).
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Follow work assignments and schedules in completing assigned tasks.
  • Cooperate with inter-departmental personnel and other facility personnel to maintain nursing services.
  • Notify facility when late or absent from work.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental and mouth care, bath functions, combing hair, dressing and undressing as necessary).
  • Keep residents dry (change gown, clothing, and linens when wet or soiled).
  • Assist residents in preparing for medical tests (lab work, x-ray, therapy, dental, etc.).
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing resident for physical examination.
  • Observe and report presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
  • Report injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents; record and report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify Nurse Supervisor/Charge Nurse of any resident leaving/missing from facility.
  • Use only equipment trained to use; report defective equipment to Nurse Supervisor/Charge Nurse.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident's refusal of treatment request; report such requests to supervisor.

Requirements

  • Minimum 10th grade education.
  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
  • CPR preferred.

Skills

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to solve practical problems and deal with concrete variables where limited standardization exists.
  • Ability to interpret instructions furnished in written, oral, diagram, or schedule form.

Physical demands

  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office related equipment (copier/scanner/fax, telephone, calculator).
  • May be necessary to assist in evacuation of residents during emergency situations.

Work environment

  • Noise level is usually low to moderate.
  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
