About the role

The primary purpose of this position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as directed by supervisors.

Responsibilities

• Identify residents using wristband or photo card before administering treatments or serving meals.
• Use only authorized facility abbreviations when recording information.
• Report all changes in a resident's condition to the Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, or charts in an informative, descriptive manner.
• Report all accidents and incidents observed during the shift immediately.
• Agree not to disclose residents' protected health information and report suspected violations.
• Report any known or suspected unauthorized attempts to access facility information systems.
• Perform only nursing care procedures for which you have been trained.
• Prepare residents' rooms for receiving (e.g., make beds, place name tags, provide admission kits).
• Greet residents, escort them to their rooms, and introduce them to roommates and personnel as appropriate.
• Assist with inventorying, marking, and storing residents' personal possessions.
• Assist residents with packing personal possessions during transfers or discharges.
• Transport residents to new rooms or receiving areas and assist with loading/unloading from vehicles.
• Maintain a warm, positive, and calm environment on the unit and shift.
• Meet regularly with shift nursing personnel to identify and correct problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports as instructed.
• Follow established policies for exposure to blood/body fluids.
• Make beds and add extra covers as requested.
• Measure and record residents' temperatures, pulses, and respirations (TPRs).
• Weigh and measure residents as instructed.
• Frequently check residents unable to call for help and answer resident calls promptly.
• Ensure residents' personal care needs are met and assist with identifying food arrangements for those with sight issues.
• Record residents' food and fluid intake and report changes in eating habits.
• Keep residents' water pitchers clean, filled with fresh water, and within reach on each shift.
• Perform after-meal care (e.g., remove trays, clean residents' hands, faces, and clothing).
• Assist residents with daily functions (e.g., dental/mouth care, bathing, hair care, dressing/undressing).
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents in preparing for medical tests (e.g., lab work, x-ray, therapy, dental).
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Perform restorative and rehabilitative procedures as instructed.
• Prepare residents for physical examinations and check routinely to ensure personal care needs are met per their wishes.
• Observe and report pressure areas, skin breakdowns, skin tears, and injuries of unknown source.
• Observe disoriented and comatose residents and report data as instructed.
• Perform special treatments as instructed.
• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
• Use only equipment for which you have been trained and report defective equipment.
• Maintain confidentiality of all resident care information and report unauthorized disclosures.
• Report all allegations of resident abuse and/or misappropriation of resident property.
• Honor residents' refusal of treatment requests and report such requests to supervisors.

Qualifications

• Minimum education: 10th grade education.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
• Ability to solve practical problems and deal with concrete variables in situations with limited standardization.
• Ability to interpret instructions furnished in written, oral, diagram, or schedule form.
• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred.

Physical Demands

• Occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computer.
• Regularly required to sit, stand, walk, and talk; read or hear.
• Frequent use of office equipment (e.g., copier/scanner/fax, telephone, calculator).
• May assist in evacuating residents during emergencies.

Work Environment

• Noise level in the work environment is usually low to moderate.
