About the role
The CNA provides personal care and other services outlined in the Plan of Care to patients/clients.
Responsibilities
- Assist patients by providing personal services, such as, bathing, dressing, and grooming as outlined on the plan of care.
- Helps family members care for the patient by demonstrating appropriate ways to lift, turn, and re-position the patient.
- Aid families in maintaining their normal and usual activities of life.
- Encourage activities of daily living while promoting independence.
- Observe for incidental needs of the patient/client.
- Exercise initiative and judgment in performing duties.
- Maintain charting and records as required by the agency and federal and state regulations.
- See referred patients as directed by the Case Manager.
Qualifications
- Graduation from high school or completion of GED preferred, but not required.
- Certified Nurses Aide in good standing - required for Help at Home program.
- Basic level knowledge of EMR (electronic medical records) and email functions.
