About the role

The Certified Nursing Assistant (CNA) functions in an assistive role in the provisions of patient care services through regular assignments or delegated tasks and is supervised by the RN. The CNA demonstrates the ability to carry out specific tasks with reasonable skill and safety that adheres to the prevailing standard of practice in the nursing community.

Responsibilities

• Provide care in accordance with Physician’s orders/Plan of Care/Home Health Aide Assignment Sheet.
• Document all tasks completed, including checking/documenting patients’ pulse, temperature and respiration.
• Assist patients in simple prescribed exercises and range of motion.
• Maintain records in order to report and discuss observations with a supervisor or case manager.
• Help patients move in and out of beds, baths, wheelchairs or automobiles, and assist with dressing and grooming.
• Change bed linens, wash and iron patients’ laundry, and clean patients’ quarters.
• Perform duties as requested by client, including running errands and preparing meals.
• Function as a team member demonstrating collaboration with and responsibility to the patient, Physician and the health care team.
• Communicate observations of significant changes in patient status to the RN Case Manager.
• Attend Multi-Disciplinary Team Meetings and document attendance via sign-in sheets.
• Communicate to RN Case Manager/Director of Clinical Services for each patient as needed.
• Participate in Agency’s mandated activities such as InServices.
• Support and execute the mission, ethics, and goals of the company effectively.
• Complete all clinical documentation accurately and submit timely per agency policy/guidelines.
• Represent the company in a positive and professional manner in the company and community.
• Adhere to dress code with a clean and neat professional appearance.
• Report on time and as scheduled in order to complete work within designated time.
• Adhere to all company policies and procedures outlined in the Employee Handbook, Employee Agreement, or communicated from executive team.

Requirements

• Graduation from an approved certification/program/accredited school.
• Current, unencumbered state certification to practice as a Certified Nursing Assistant (CNA).
• Professional experience required per state regulations to perform the job as described.
• Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
• Provide copy of current TB results or Chest X-Ray.
• Provide copy of current CPR.
• Be flexible, organized and have the ability to exercise sound judgement to perform the job duties/responsibilities as described.

Physical Requirements

• Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms and other physical activities as deemed necessary by job duties.
• Vision and hearing capabilities within normal limits, with or without aid.
• Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable.
• Ability to perform the anticipated level of physical assist required to provide safe client care. If the anticipated level of assist is greater than that which the employee can safely provide, the supervisor is to be notified immediately.

Work Environment

The work environment is highly variable, as care is generally provided in private homes. Lighting, acoustics, air quality, temperature and environmental hazards are not predictable. The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals. The employee is required to travel frequently to complete home health visits.
