Our Summit ElderCare® is a Program of All-Inclusive Care for the Elderly (PACE), an alternative to nursing home care that helps people 55 and older continue living safely at home. Responsibilities

  • Provide personal care, light housekeeping, and assistance with Activities of Daily Living (ADLs) as outlined in each participant’s plan of care, both at the PACE center and in participants’ homes (community, Assisted Living Facilities, Rest Homes, Supportive Housing programs, etc.).
  • Contribute to the development of care plans through collaboration with the Interdisciplinary Team (IDT).
  • Carry out non-skilled treatments, including vital signs, transfers, toileting, bathing, and dressing.
  • Assist with individual and group recreational programs, including planning, participant motivation, and program evaluation.
  • Support registered therapists and certified therapy assistants with participant-specific activities such as positioning, transfers, ambulation, and exercises.
  • Maintain a clean and safe environment at the PACE center and in participants’ homes.
  • Assist with meal and snack preparation, serving, feeding, and clean-up.
  • Use safe techniques in all interactions with participants.
  • Provide accurate and timely documentation in the Electronic Medical Record (EMR) and other systems.
  • Participate in daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines.
  • Actively distribute and manage work assignments to ensure timely completion of participant care and site tasks.
  • Participate in regular team meetings for health aides.
  • Follow infection control precautions and CDC guidelines for cleaning and disinfecting.
  • Perform all duties in accordance with Fallon Health and Summit ElderCare policies and procedures.

Requirements

  • Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but equivalent work experience may be accepted in lieu of certification.
  • Certification as a Home Health Aide or Nursing Assistant is desired.
  • Reliable transportation is required.
  • One year of experience working with a frail and/or older adult population, preferably in a PACE program. If under one year, completion of Summit ElderCare’s training program for health aides is required before working independently.
  • Annual completion of a standardized set of Summit ElderCare competencies for Health Aides.

Pay

In accordance with the Massachusetts Wage Transparency Act, the pay for this position is $21.50 per hour on weekdays and $24.00 per hour on weekends. Final compensation will depend on the candidate’s experience, skills, and fit with the role’s responsibilities.
