CNA/HHA Relief (ABI)

Essential Duties and Responsibilities

• Maintain respectful communication and interactions with consumers at all times.
• Ensure that individuals have the resources needed to make informed choices and are active participants in decisions affecting their lives and supports received.
• Afford assistance with personal care needs and skill training.
• Follow all diet protocols as ordered by a physician.
• Report any suspicions of abuse or neglect to the Disabled Persons Protection Commission mandated reporter.
• Report any injuries or potential health problems to the program nurse.
• Afford assistance with daily needs and ensure that all activities are reflective of their choices.
• Take initiative to integrate individuals into the community in areas of his/her choice.
• Afford assistance in meeting goals identified in all areas of the ISP/DHSP complete and documentation as indicated.
• Expose individuals to new ideas and choices, which assist in their continued growth and development.
• Is responsible for the safety and well-being of all individuals.
• Is aware of and follows all Individual and Program Specific Safety Plans and approved behavior program.
• Ability to carefully monitor and supervise individuals served.
• Is prepared for and actively participates in all mandatory meetings.
• Demonstrate flexibility in meeting program needs.
• Respects the role and responsibility of his/her supervisor and maintains the same work diligence whether his/her supervisor is present or not.
• Maintain a courteous, professional attitude while conducting business by effectively communicating with all LifeStream personnel, family members, and all community contacts.
• Completes all assigned tasks in a timely manner with minimal assistance.
• Maintain cleanliness of program areas.
• Avoids bringing personal matters to work that could have a disruptive impact on the consumer or co-workers.
• Consistently advocates for the individuals served in all areas above.

Qualifications and Requirements

• 18 Years of age
• Valid Driver’s license
• Authorized to work in the US
• Once hired, must attend LifeStream orientation and trainings
• Access to a vehicle
• Education and/or Experience:
• Demonstrated history of quality work performance and teamwork in previous positions
• High School Diploma or GED or CNA/HHA
• One year experience in the field of Human Service
