CNA/HHA Certified Nursing Assistant

About the Role

The Certified Nursing Assistant (CNA) functions in an assistive role in the provision of patient care services through regular assignments or delegated tasks and is supervised by the RN. The CNA demonstrates the ability to carry out specific tasks with reasonable skill and safety that adheres to the prevailing standard of practice in the nursing community.

Responsibilities

• Check and document patients’ pulse, temperature, and respiration.
• Assist patients in simple prescribed exercises and range of motion.
• Maintain records to report and discuss observations with a supervisor or case manager.
• Help patients move in and out of beds, baths, wheelchairs, or automobiles, and assist with dressing and grooming.
• Change bed linens, wash and iron patients’ laundry, and clean patients’ quarters.
• Perform a variety of duties as requested by the client, such as running errands and preparing meals.
• Function as a team member, demonstrating collaboration with and responsibility to the patient, physician, and healthcare team.
• Follow shifts or visits according to the orders on the Plan of Care/Home Health Aide Assignment sheet.
• Communicate observations of significant changes in patient status to the RN Case Manager.
• Attend Multi-Disciplinary Team Meetings as recommended and document attendance via sign-in sheets.
• Communicate with RN Case Manager/Director of Clinical Services for each patient as needed.
• Participate in the agency’s other mandated activities, such as in-services, as requested by the Agency Director.
• Support and execute the mission, ethics, and goals of the company effectively.
• Complete all clinical documentation accurately and submit it timely per agency policy/guidelines.
• Represent the company in a positive and professional manner in the community.
• Adhere to the dress code with a clean and neat professional appearance.
• Report on time and as scheduled to complete work within the designated time.
• Adhere to all company policies and procedures outlined in the Employee Handbook, Employee Agreement, or communicated by the executive team.

Requirements

• Graduation from an approved certification program, accredited school, or relevant post-graduate education to reflect the ability to perform the job as described.
• Current, unencumbered state certification to practice as a Certified Nursing Assistant (CNA).
• Professional experience required per state regulations to perform the job as described.
• Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
• Provide a copy of current TB results or Chest X-Ray.
• Provide a copy of current CPR certification.
• Be flexible, organized, and able to exercise sound judgment to perform job duties and responsibilities.

Physical Requirements

• Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms, and perform other physical activities as deemed necessary by job duties.
• Vision and hearing capabilities within normal limits, with or without aid.
• Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more, if applicable.
• Ability to perform the anticipated level of physical assist required to provide safe client care. If the anticipated level of assist exceeds the employee’s safe capacity, the supervisor is to be notified immediately.

Work Environment

The work environment is highly variable, as care is generally provided in private homes. Lighting, acoustics, air quality, temperature, and environmental hazards are not predictable. The employee may occasionally be exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals. The employee is required to travel frequently to complete home health visits.
