Essential Duties & Responsibilities

• Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice
• Collections data for ongoing interdisciplinary comprehensive assessments
• Participates in the development and evaluation of the plan of care by reporting observations
• Contributes to patient and family counseling and education
• Performs duties in compliance with Medicare Condition of Participation
• Practice safety and infection control methods
• Provides hands-on personal care
• Works to achieve performance goals, pursue progress toward individual development plan (IDP), and demonstrate Four Seasons values and competencies

Education

• Minimum Qualifications: High School Diploma/GED
• No more than a 24 month lapse in providing clinical care services upon the completion of a State-Accredited Certified Nursing Assistant course
• 0 – 1 year of clinical experience

Licenses & Certifications

• (if applicable): Valid NC CNA certificate
• Valid driver’s license and proof of automobile insurance
