Essential Duties & Responsibilities
- Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice
- Collections data for ongoing interdisciplinary comprehensive assessments
- Participates in the development and evaluation of the plan of care by reporting observations
- Contributes to patient and family counseling and education
- Performs duties in compliance with Medicare Condition of Participation
- Practice safety and infection control methods
- Provides hands-on personal care
- Works to achieve performance goals, pursue progress toward individual development plan (IDP), and demonstrate Four Seasons values and competencies

- Willingness and ability to care for patients with infectious disease processes unless approved accommodations

Education
- Minimum Qualifications: High School Diploma/GED
- No more than a 24 month lapse in providing clinical care services upon the completion of a State-Accredited Certified Nursing Assistant course
- 0 – 1 year of clinical experience
Licenses & Certifications
- (if applicable): Valid NC CNA certificate
- Valid driver’s license and proof of automobile insurance
