About the role

Ennoble Care is a mobile primary care, palliative care, and hospice service provider serving patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia. Our clinicians provide care in patients' homes, focusing on those with chronic conditions and limited mobility. We offer services like remote patient monitoring, behavioral health management, and chronic care management to ensure high-quality care by trusted teams.

Responsibilities

• Care for a census of 5-6 patients
• Provides or assists in activities of daily living (ADLs): personal hygiene, grooming, mouth care, skin care, and nails
• Assists with toileting, positioning, and transfer activities, frequent turning, and positioning of bed-bound residents/patients
• Assists with prescribed exercises and range of motion (ROM) activities taught by the Hospice Nurse
• Assists with light housekeeping of individual's personal unit or room, including changing bed linen and patient laundry
• Feeds and assists with meals
• Reports any changes in resident's/patient's condition to the Nurse

Requirements

• Must have a valid PA CNA
• Valid Driver’s License and reliable mode of transportation
• Authorized to work in the United States
