Responsibilities
- Provide patient-centered care under the supervision of a Registered Nurse
- Complete proper documentation using HomeCare HomeBase EMR
- Assist clients with either personal care or home management tasks, including assistance with basic personal hygiene and grooming, feeding and ambulation, medical monitoring, and other basic health care related tasks; home management includes basic housekeeping, shopping and other related housekeeping tasks
