About the Role
Position is for a small 40-bed Memory Care Community, available full-time (FT), part-time (PRN), and for all shifts.
Responsibilities
- Assist residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene, and mobility
- Encourage socialization among residents
- Assist with the activity program; conduct activities as directed by the Activity Director or assigned by supervisor
- Monitor health, safety, and well-being of the residents
- Document information pertaining to residents' functional status or condition in appropriate flow sheet binder
- Provide hands-on support and eye-level observation to residents
- Assist in keeping the environment safe for residents
- Report any changes or concerns regarding residents' condition to the Director of Nursing (D.O.N.) and/or Nurse on staff
- Maintain an accurate record of resident meals intake, output, and bathing record
- Do personal laundry for residents
- Carry out other duties as assigned by the Supervisor
- Attend and participate in monthly meetings
- Read all pertinent resident information in the staff communication log, daily progress notes, and flow sheets
Requirements
- Strong background in memory care is recommended but will train the right person
