About the Role
Provides personal hygiene services, assists with ambulation and exercises as defined by therapist, and performs simple household services essential to maintain the health and well-being of the patient. Assists with medications that are normally self-administered. Reports changes in the patient's condition and needs to the supervising RN.
Qualifications
- High School Diploma or GED (Required)
- Completion of Home Health Aide training program (Required)
- 1–2 years of experience in an institutional healthcare setting assisting patients with ADLs (Preferred)
Certifications/Licensures: Certified Nursing Assistant (CNA) (Required)
Responsibilities
Care Processes
- Treats patients and families with compassion and implements the plan of care
- Bases care decisions on the plan of care
- Monitors and documents patient response
- Establishes rapport with clients and adapts communication style to meet patient needs
- Recognizes age-specific responses to treatment and provides effective interventions
- Respects patient confidentiality and privacy
- Supports social, spiritual, and cultural needs of patients and families
- Manages difficult or emotional patient situations
Documentation
- Documents and submits required information and data in a timely fashion
- Clearly and accurately documents processes, policies, and service offerings
- Ensures documentation is tailored to expected readers/users
- Uses correct terminology and conforms to required style and format
Technology and Equipment Use
- Demonstrates technical skill in using equipment appropriate for the role
- Understands equipment operation and maintains it in working order
- Troubleshoots equipment problems
- Uses technology to increase productivity and adapts to new technology
Additional Duties
- Adheres to hospital and departmental attendance and punctuality guidelines
- Performs all job responsibilities in alignment with the organization’s core values, mission, and vision
- Performs other duties as required and completes all job functions per departmental policies and procedures
- Maintains current knowledge in present areas of responsibility through self-education and ongoing programs
- Attends staff meetings and completes mandatory in-services, requirements, and competency evaluations on time
- Demonstrates competency in providing care to all patients based on age, sex, weight, and demonstrated needs
- For non-clinical areas, attends training and demonstrates age-specific customer service skills
- Wears protective clothing and equipment as appropriate
