As a CNA, you will provide direct, hands-on care to residents while assisting with activities of daily living, mobility, personal care, and other essential needs. You will work closely with nurses and other members of the interdisciplinary team to help ensure each resident receives safe, respectful, and individualized care. Our CNAs are an essential part of our clinical team. This position is ideal for someone who enjoys building relationships with residents, working as part of a team, and making a meaningful difference during every shift. Key Responsibilities Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and personal hygieneObtain and accurately record vital signs, including temperature, pulse, respirations, and blood pressureFollow resident-specific care plans and communicate concerns or changes to licensed nursing staffMaintain resident rooms and care areas in a clean, safe, and organized conditionFollow infection prevention, fall prevention, transfer, skin integrity, and resident safety proceduresProtect resident privacy, dignity, independence, preferences, and rightsCommunicate respectfully with residents, families, visitors, nurses, and interdisciplinary team membersFollow all facility policies and applicable federal and Wyoming requirements Qualifications Current, active Certified Nursing Assistant (CNA) certification in the State of WyomingCertification must be in good standing with no disqualifying findings on the applicable Nurse Aide RegistryPrevious experience in skilled nursing, rehabilitation, long-term care, assisted living, or another healthcare environment preferred but not requiredStrong communication and interpersonal skillsAbility to follow individualized resident care plans and nursing directionDependable and able to work effectively as part of a clinical teamStrong observation skills and ability to recognize and report changes in resident conditionAbility to prioritize responsibilities in a fast-paced healthcare environmentCommitment to providing compassionate, respectful, and resident-centered careAbility to perform the essential functions of the position with or without reasonable accommodation Benefits $2,500 Sign-On Bonus for eligible full-time CNAsCompetitive pay401(k) retirement planHealth, dental, and vision insurancePaid Time Off (PTO)Care Cash ProgramEmployee Referral Bonus ProgramTraining and professional development opportunitiesOpportunities for career advancementSupportive and collaborative work environment Grow With Us At Polaris Rehabilitation & Care Center, we believe in developing our team from within.
