About the role
Polaris Rehab is seeking a dedicated and compassionate Certified Nurse Aide (CNA) to provide direct, hands-on care to residents in a skilled nursing setting. This role is essential in supporting residents with activities of daily living while promoting dignity, safety, and overall well-being.
Key Responsibilities
- Accurately obtain, record, and report vital signs, including temperature, pulse, respirations, and blood pressure, escalating any abnormalities to licensed nursing staff
- Assist residents with mobility, including safe transfers, ambulation, and repositioning in alignment with care plans and safety protocols
- Provide support with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and nutritional assistance
- Respond promptly and professionally to call lights and resident requests, ensuring timely and respectful care delivery
- Maintain a clean, safe, and organized environment consistent with infection control and safety standards
- Observe, document, and report changes in resident condition or behavior to the supervising nurse in a timely manner
- Communicate effectively and professionally with residents, families, and members of the interdisciplinary team
- Adhere to all facility policies, as well as applicable state, federal, and infection prevention regulations
Qualifications
- Active Certified Nursing Assistant (CNA) certification in the State of Wyoming, in good standing with no disqualifying findings on the Nurse Aide Registry
- Demonstrated ability to collaborate effectively within a clinical team environment
- Strong critical thinking skills and adaptability in a fast-paced healthcare setting
- Excellent communication and interpersonal skills
