About the Role
At Heritage Hills Rehabilitation & Healthcare Center, we believe that exceptional resident care extends beyond the bedside. Guided by our core values of Passion, Respect, and Excellence, the CNA Transportation Coordinator provides compassionate, hands-on resident care while coordinating medical appointments and transportation services to ensure residents receive timely access to healthcare.
The facility is a 100-bed Sub-Acute and Long-Term Care center thoughtfully designed with beautiful common spaces, creating a welcoming, home-like environment for both residents and staff. Responsibilities
- Schedule and coordinate physician appointments and other medically necessary resident appointments.
- Arrange transportation for residents to and from medical appointments while ensuring schedules are accurate and communicated appropriately.
- Coordinate with transportation providers, families, and healthcare offices to facilitate seamless resident care.
- Maintain accurate records of appointments, transportation schedules, and related documentation.
- Provide basic resident care under the direction and supervision of licensed nursing staff in a safe, compassionate, and respectful manner.
- Assist residents with activities of daily living, including feeding, bathing, dressing, grooming, mobility assistance, and changing linens.
- Provide supportive care to ensure residents' physical, emotional, and daily living needs are consistently met.
- Observe residents and promptly report changes in condition, behavior, or care needs to nursing staff.
- Maintain a clean, safe, and comfortable environment that promotes resident dignity and well-being.
- Support nursing staff and the interdisciplinary team in delivering high-quality, resident-centered care.
Requirements
- Must hold a current, active, and unencumbered Certified Nursing Assistant (CNA) license in the state of Rhode Island.
- Valid Basic Life Support (BLS) / CPR certification required.
- Previous experience scheduling medical appointments and coordinating resident transportation is required.
- Ability to work effectively with multiple residents in a fast-paced, team-oriented healthcare environment.
- Demonstrated compassion, patience, and commitment to providing high-quality resident care.
- Strong communication, organizational, and scheduling skills.
- Ability to coordinate multiple appointments while maintaining accuracy and attention to detail.
- Must have a clean driving record.

Pay
$20-$23 an hour (all-inclusive), with daily pay available.
Schedule
Full-time, 8 am–4 pm.
